Six-digit MFA form for Nile sign-in challenges (authenticator or email OTP).

Used on /auth/mfa during sign-in. login() navigates here with ?token=… and, when Nile sends it, ?method=email or authenticator.
Requires xAuth.features.mfa. Off → the page redirects to login.

On success it navigates to afterLogin. No token → submit disabled, “sign in again”. Code must be 6 digits.
Email MFA codes are not signup verification emails.
